Discharge Transfer from WGH to Community Hospitals

- · Discharge Transfer functionality in Discharge Plan can be used for inpatients transferring from WGH to a community hospital.
- · Discharge Transfer allows the sending physician to suggest orders for the receiving physician. The receiving physician can then review the suggested orders and decide to continue or omit the orders.
- With this change, Prescriptions and Problem Reconciliation are not required with a Discharge Transfer – once the discharge disposition is selected by nursing, the required red asterix will be removed.
- Tip sheets will be posted here prior to go live.

- The Anesthesia Pre Op Pediatric order set will be updated to include a Medical Directive order for pre-op analgesia. The order will flow to the worklist for nursing to enter the Medical Directive at the time of visit when a weight has been documented.
- An additional communication will be sent to surgical services regarding this change.

To ensure that PKU’s on newborns are not missed, the Newborn order sets have been updated so that these orders default to lab to collect.

Personalized Order Sets Pop Up
To turn off the notification that a personalized order set has been updated go to Menu>Manage Personal Sets, select the order set and click “Do Not Show Again”.
Admission Cancellations in ED
When the admission of an ED patient has been cancelled, please ensure that the Admit Request order is cancelled. Nursing will follow up with Registration to have the account corrected.
To ensure Health Records and CIHI standards are met, the documentation must accurately reflect the account type. If an Admission Note has been started it will have to be adjusted to an ED Report.

Barriers to Discharge in Discharge Summary
Starting December 5th, a change will be made to the Barriers and Goals section in the Discharge Summary. During this transition, you may temporarily see multiple responses for barriers prior to and after December 5th.
Please note that the Barriers to Discharge section is view only and will not display on the Discharge Summary output.

Cancel All prompt for orders

- Some order sets (e.g. Admit Med/Surg) have the setting “Cancel All” associated with them. As a result, when cancelling an individual order from that order set, it prompts to cancel all the orders.
- This change will turn off Cancel All function for all ED, Admit and Convenience order sets. The order set PCA, DKA Fluids, Midazolam and Naloxone will still have Cancel All associated with them
- This will reduce clicks and improve order set usability when physicians are stopping orders placed from within the order set.

Suppress Titrate

- The titrate button is currently available within infusions (I.e., PCA) when accidentally de-selected removes the protocol attached to that drug string. Manually replacing the protocol into the string is the only option after it is selected, which has resulted in protocols being removed from high alert medications and infusions
- Other sites have suppressed this setting to ensure protocols are not removed from infusion strings
- Eliminate the potential for the protocol to be removed and does not impact the actual ordering of a titrating infusion

Beta-hydroxybutyrate testing
- Effective July 28, beta-hydroxybutyrate testing will replace serum ketones testing and will be available on site at the Whitehorse General Hospital Laboratory using whole blood samples sent to the laboratory for measurement with the Nova Statstrip Hospital Point of Care Meter™. Beta-hydroxybutyrate testing will not have to be sent to SPH for processing, improving TAT for results aiding with diagnosis and treatment of patients at WGH. No extra order sets to be built, clear definite testing for all order sets currently using Ketone and B-hydroxybuterate)

Plexia Report Distribution Issue
- On July 12, a critical issue was reported to the 1Health team related to report distribution from MEDITECH to Klondyke Medical Clinic
- The issue was due to an error within Plexia
- The issue was resolved by Plexia on the evening of July 13

- The bundled CBC w BMP and CBC w CMP orders create issues in lab. This change would inactivate the bundled orders and use the two standalone orders instead. Part of this change would also fix the Lab Basic Metabolic Panel build and Comprehensive Metabolic Panel build on the Lab side so that the results flow to Plexia correctly.
- A CBC w BMP convenience order set and a CBC w CMP convenience order set will be created.
- Impacted order sets will be updated with the two standalone orders.
- Patients no longer have a second phlebotomy done unnecessarily
- Reduces chance of delayed testing/results
- Reduced workload for Lab

- CT orders: Fields have been reduced, After Hours Radiologist and Protocol have been combined with Comment field.
- Community Hospitals: Imaging orders completed at WGH will be available under Ambulatory Orders
- More space to enter clinical indicators
- Less clicks for imaging orders in the ED
- Streamlined ordering with fewer questions to answer

This simplifies the information in the Activity Log. All information is still available in the patient’s chart.

Flowsheet Requests
Flowsheet requests can come through JIRA and will include a physician lead assisting with the development. Flowsheets will go to the relevant committee and PLC for approval.

A request (JIRA 12685) was submitted to have the sign queue orders not pre-selected. This was reviewed at PLC and a decision was made to leave the sign queue as is and not implement the requested change.

Updating Collection Defaults - "Nurse to Collect" or "Lab to Collect"
- There are several lab orders in the LIVE Meditech system that have been identified to have the incorrect collection defaults.
- Once these collection defaults are corrected, nurses and physicians will no longer need to change the default collection field when the order is placed.
Example: an HPV sample is normally collect by a Nurse and then sent to the lab. Currently, the default is “Lab to collect”, therefore the nurse has to enter the order and change the collection default manually to correct it. Once the change is implemented, when an MD orders HPV it will flow to the worklist and default to “Nurse to collect”.
- This change has been approved by YHC lab, the order management team, and facilitated by 1Health to streamline the lab ordering process and reduce human errors.
- The work to implement these changes will commence in MEDITECH on May 3rd.
